In collaboration with patient/family, social workers, physicians and interdisciplinary team, we are seeking a Clinical Social Worker who will provide direct psychological, social and practical care including social work assessments, problem identification, goal setting and implementation of care plans. Collaborates with all disciplines to optimize patient outcomes; scope of services may include, but is not limited to, developing action plans to mitigate psycho-social barriers to discharge, patient/family engagement and education, and employing interventions (crisis intervention, grief counseling, suspected abuse reporting) to address immediate needs.
This position provides clinical social work services in the Post-Acute COVID-19 Team (JH PACT) outpatient clinic at Johns Hopkins Bayview Medical Center. This clinic is a collaboration between the Division of Pulmonary & Critical Care Medicine and the Department of Physical Medicine and Rehabilitation. The goal is to address the unique needs of COVID-19 survivors as an inter-disciplinary team. The Clinical Social Worker is responsible for psychosocial assessment and clinical interventions including behavioral health needs, coping and adjustment to illness and hospitalization, crisis intervention, non-pharmacological symptom management, family meetings, and community resource referrals, etc. Includes collaboration with rehabilitation psychologist, psychiatrist, and community health worker. May include facilitation of a peer support group.
